Out Patient Procedure

When coming to the hospital for the first time you can do online Registration through our website. This will reduce procedural delay during registration. Try to bring a responsible attendant along with you who will assist you and if need be take decisions on your behalf. You are requested to carry Aadhar card (self and spouse or parents) while visiting the hospital. This will be required at the time of Registration.

Bring a referral note, medical documents and medicines relating to your present and past medical conditions (such as diabetes, hypertension, asthma etc.) and operations (e.g. angioplasty, bypass surgery, etc.) in addition to those directly related to your current medical condition (Biopsy reports, blocks, slides, CT scans, blood investigations etc.)

If you are an outstation patient, it may be beneficial to check whether your preexisting medical conditions such as hypertension (high blood pressure), diabetes, etc. are controlled. This can be done while you are planning your visit to Mumbai. This may avoid delay in treatment and reduce the duration of your stay in Mumbai.

The Hospital working hours are 9.15 a.m. to 5.30 p.m. from Monday to Friday. Working hours close at 2.15 p.m. on Saturdays. The Hospital Registration and Outpatient services are closed on all gazetted Central Government holidays. Emergency services are available round the clock daily.

Outpatient services are currently available only for select Oncology specialties at ACTREC and consist of:

  • Hematolymphoid
  • Pediatric
  • Breast
  • Head and Neck
  • Neuroncology
  • Gynecology (pertaining to Radiation and Chemotherapy)
Registration

Registration is carried out at the Front Office located on the ground floor of the Paymaster Shodhika. You may choose to register as a General or Private patient depending upon your financial capacity to pay for the services that you will avail. You are suggested to make an informed choice as transfer from one category to another is permitted only once.

You are required to fill in a personal particulars form that will include your demographic details. Please enter correct information, in order to avoid any inconveniences during documentations for reimbursements/ claim settlements/ travel concessions etc. If you have any difficulty please seek the help of Front Office staff/Medical Social Worker/Voluntary Social Workers. After filling the form the Front office will take a patient photograph at site and issue a case file with unique case file number (Alpha numeric number) and a smart card. This Registered case file number will be your unique identity for all your medical records, your account and the services you will avail from the hospital.

Smart card

Smart card bearing your name, registered case file number and photograph will be your unique identity card required to be produced each time to avail consultation, lab tests, imaging and other clinical services in the hospital. A smart card account will also be opened with your Registered case file number and you will be requested to load sufficient amount as deposit and need to carry the Smart card with you all the time to avail patient services in the hospital (without smart card patient billing will not be possible through the smart card account). Also, it enables speedy retrieval of the medical case files and details from the Hospital Information system. The case file number is an unique identifier of your medical records.

General patients

The Medical Social Worker will interview general patients and depending on the patient's financial capacity will be assigned to ‘C’ or ‘NC’ categories. The General 'C' category patients will be required to pay very nominal charges for services availed while ‘NC’ category patients won’t require to pay charges for most of the services, as per the Schedule of Charges. If you are referred by your employer/insurer having prior credit arrangement with Tata Memorial Centre / ACTREC please contact our Accounts Department for extension of credit facility.

Private Patients

As a Private patient, you may choose your Doctor. In case you do not have a choice, the authorities will assign you a Doctor appropriate for your case. If you are referred by your employer/insurer having prior credit arrangement with Tata Memorial Centre / ACTREC please contact our Accounts Department for extension of credit facility.

Consultation

After registration you will be directed to your treating Doctor/Service. The Doctor will review the medical reports and may ask you to undergo further diagnostic evaluation/ investigations.

You may also be referred to other Doctors in the hospital for specialized consultation. It is advisable to undergo all investigations at our hospital or at Tata Memorial Hospital, Parel whenever requested.

Diagnostic Services

Based on the advice of your treating Doctor you will be directed to various diagnostic areas where you may be required to give blood/body fluid samples. If required you may also have to undergo diagnostic procedures/biopsies.

As an outpatient you will be required to pay upfront for all the services that you avail. A consolidated memo will be prepared at the respective counter for payment with your smart card. If you are a credit patient, the charges towards the services availed will be debited from your account.

The treating Doctor after evaluating the reports will discuss the treatment options with you. Please clarify all issues of concern arising out of the treatment, if any. In the event that the Doctor suggests that you need to take treatment as an inpatient, please inquire whether you need to arrange for blood/bone marrow donors. Your Doctor will also give you an estimate of the cost of treatment. Please note that cost of treatment may increase in the unlikely event of complications for which you have to make provisions.

On completion of your evaluation, you are required to deposit the case file at the OPD counter. You may photocopy the contents for your reference and records. Attestation of photocopies of patient case files will be done by the Medical record Officer.

Appointment & Travel Concession

For your next visit, your Doctor's secretary or the Registration Office will issue an appointment slip for you, with details such as date and time of the next appointment. You may also choose to avail travel concession for Road/Rail/Air transport to your place of permanent residence declared during registration, as per rules laid down by the concerned travel authorities.

Financial Assistant

Mandatory Documents to avail Financial Support from Government Schemes (MPJAY, PMJAY- Ayushman Bharat)

Those Employed with Organized and Unorganized Sectors

  • Income proof issued by the state (Tahsildar / Sub Divisional Officer /Block Development Officer).
  • Ration card issued by Rationing office of the state.
  • Aadhar Card or valid photo identity issued by government as driving license, passport, voter ID, pan card, other identity cards which may be issued by employers.
  • Pradhan Mantri Jan Arogya Yojana (PMJAY) - Ayushman Bharat Card.
  • Beneficiary cards issued by Respective State Governments for State Health Schemes.
  • Salary slip
  • ESIS Card/CGHS Card/Referral letter from concerned officials recommending treatment at Advanced Centre for Treatment, Research and Education in Cancer (ACTREC). Details of mediclaim and insurance applied by self or employer may please be disclosed and shared.

Please Note: For quick accesses to services kindly furnish documents which have a uniform name, address on all above said documents. Discrepancy if any might lead to delay or losing access to benefits.

Hospital Pharmacy

The Hospital has a well-stocked Pharmacy with in the premises. We ensure that quality medicines/consumables at reasonable rates are sold through our Pharmacy across all categories of patients. It is therefore prudent to procure medicines/consumables from the Hospital Pharmacy itself. The Pharmacy can accept return of unused medicines, costing more than Rs. 100/- per memo, up to 1 months and refrigerated medicines within 24 hours from the date and time of sale, provided that the integrity of the medicines is satisfactory.

Day Care Centre

The Day Care Centre for both General and Private patients is functional during the day for patients requiring chemotherapy/blood products infusion/intravenous fluids.

Working hours: Monday – Friday: 7.30 am to 9:00 pm
Saturdays & Sundays: 7.30 am to 5:00 pm
